TESTIMONIAL CONSENT AND RELEASE FORM
I authorize Artera to publish the testimonial below (“Testimonial”) in any of Artera's marketing materials including, but not limited to, its website, case studies, published articles, and press releases.  I understand that my name, title, and company will be disclosed with my Testimonial. In giving my Testimonial, I represent that I am an authorized representative of my company and that I have the power and authority to grant this consent and release. I understand that Artera has the sole right to decide whether to use, or not use, my testimonial and may summarize or shorten it as needed.  I acknowledge that I may revoke this consent and release by sending a written notice delivered in writing to the Marketing team at cxmarketing@artera.io, and that my testimonial will be removed from all marketing materials and uses as soon as practicable following Artera’s receipt of my notice. I acknowledge that it may be difficult or impossible to remove it from all sources.  In both my individual capacity and my representative capacity on behalf of Artera, I hereby irrevocably waive any claim against Artera relating to the use of my Testimonial and promise not to bring any such claim or action in the future.
